Holy Spirit Parents,

If your child is going into 5th, 6th, 7th or 8th grade and wants to try-out for cheer leading or any school sports team this year, they need to have a physical done ON THE DIOCESE PHYSICAL FORM dated after June 1st.

We have arranged for these physicals to be performed right here at our school FOR FREE on July 31st from 9:00-4:00 by the St. Vincent's Mobile Health Outreach Ministry.  You may also pick up the correct form from our office and have your own doctor's office complete the physical.
